Introdução:A estética do sorriso é um aspecto valorizado pela sociedade atual, o que demanda que os profissionais de odontologia estejam atualizados sobre materiais dentários e novas técnicas para alcançar resultados estéticos restauradores de alta qualidade. A resina composta é frequentemente utilizada para simplificar a reabilitação oral, pois oferece facetas dentárias que combinam excelente estética e durabilidade. Objetivo:Este artigo tem como objetivo relatar um caso clínico de reabilitação estética utilizando facetas em resina composta com fechamento de diastemas.Relato de caso:Uma paciente do sexo feminino, com 24 anos de idade, procurou a clínica escola de odontologia com insatisfação em relação à estética do seu sorriso. A pacienteapresentava diastemas, restauração defeituosa e desproporção no tamanho dos dentes anteriores. O tratamento consistiu em um clareamento caseiro prévio, seguido pela confecção de facetas diretas em resina composta para fechamento dos diastemas e correção da desproporção dentária. Foram utilizados um Jig Estético e o software Digital Smile Design (DSD)para o planejamento, além de um mock-up em resina bisacrílica. Após a confecção das facetas, foi obtido um resultado estético final satisfatório. O caso incluiu um total de oito facetas,dos dentes 14 ao 24. Conclusão:O tratamento utilizando restaurações em resina composta proporcionou à paciente uma reabilitação estética, funcional e biológica (AU).


Introduction:Smile aesthetics is an aspect valued by today's society, which demands that dental professionals are up to date on dental materials and new techniques to achieve high-quality restorative aesthetic results. Composite resin is often used to simplify oral rehabilitation, as it offers dental veneers which combine excellent aesthetics and durability. Objective:This article aims to report a clinical case of aesthetic rehabilitation using composite resin veneers with diastema closure. Case report:A24-year-old female patient came to the dental school clinic with dissatisfaction regarding the aesthetics of her smile. The patient had diastemas, defective restoration and disproportionate anterior teeth size. The treatment consisted of prior at-home tooth bleaching, followed by creating direct composite resin veneers to close the diastemas and correct tooth disproportion. An Aesthetic Jig and the Digital Smile Design (DSD) software program were used for planning, in addition to a mock-up in bisacrylic resin. After treatment, a satisfactory final aesthetic result was obtained. The case included a total of eight veneers, from teeth 14 to 24. Conclusion:Treatment using composite resin restorations provided the patient with aesthetic, functional and biological rehabilitation (AU).


Introducción: La estética de la sonrisa es un aspecto valorado por la sociedad actual, hecho que demanda de los profesionales de odontología actualización constante sobre los materiales dentarios y nuevas técnicas para alcanzar resultados estéticos restauradores de altacualidad. La resina compuesta es frecuentemente utilizada para simplificar la rehabilitación oral, pues ofrece facetas dentarias que combinan excelente estética y durabilidad.Objetivo: Este artículo tiene como objetivo relatar un caso clínico de rehabilitación estética a partir de la utilización de facetas en resina compuesta con cierre de diastemas. Informe de caso: Una paciente, del sexo femenino, con 24 años de edad, buscó la clínica-escuela de odontología insatisfecha con la estética de su sonrisa. La paciente presentaba diastemas, restauración dentaria defectuosa y desproporción en el tamaño de los dientes anteriores. El tratamiento consistió en un blanqueamiento casero previo, seguido por la confección de facetas directas de resina compuesta para cierre de los diastemas y corrección de la desproporción dentaria. Fueron utilizados un Jig Estético y el software Digital Smile Design(DSD) para el planeamiento, además de un mock-upen resina bisacrílica. Tras la confección de las facetas, fue obtenido un resultado estético final satisfactorio. El caso incluyó un total de ocho facetas, de los dientes 14 al 24.Conclusión: El tratamiento a partir de restauraciones en resina compuesta proporcionó a la paciente una rehabilitación estética, funcional y biológica (AU).

ABSTRACT

This article aims to describe a clinical case of the use of the injectable restorative technique with composite resin in a patient with an esthetic complaint in the anterior teeth. Producing direct restorations in anterior teeth when performed with composite resin demands a long clinical time. Results often depend on the operator's ability to reproduce proper form, function, and esthetics. Lately, the versatility of composite resins has increased. As this material evolved, new techniques were introduced to simplify the restorative process and save time. The restorative technique with injectable fluid composite resin, in addition to delivering faster and with a certain ease, is capable of increasing the predictability of treatment results. However, it is known that flowable resins have lower wear resistance and color stability compared to conventional resins. Thus, a version of this material with a higher filler content in its composition was introduced to the market, to improve wear resistance and gloss retention. Long-term studies evaluating the use of low-viscosity resins are needed. The injectable technique can accurately reproduce the morphology obtained in the wax-up of the case, generating a satisfactory esthetic and functional result. Despite simplifying the fabrication of restorations when compared to the conventional technique, it is still necessary for the operator to have some experience and aptitude. The clinical time for the production of restorations has been considerably reduced. The material showed good flow, shine, and polish in the immediate result. However, the finishing and polishing of the restorations required a longer clinical period than usual and in the short- term follow-up, flaws in the gingival margins and a new finishing and polishing procedure were necessary. Therefore, the technique described in this article can be considered a restorative alternative, but it is still not able to replace the tradit ional method of making previous esthetic restorations.


Este artículo tiene como objetivo describir un caso clínico del uso de la técnica restauradora inyectable con resina compuesta en un paciente con una queja estética en los dientes anteriores. Producir restauraciones directas en dientes anteriores cuando se realizan con resina compuesta demanda un largo tiempo clínico. Los resultados a menudo dependen de la capacidad del operador para reproducir la forma, la función y la estética adecuadas. Últimamente, la versatilidad de las resinas compuestas ha aumentado. A medida que este material evolucionó, se introdujeron nuevas técnicas para simplificar el proceso de restauración y ahorrar tiempo. La técnica restauradora con resina compuesta fluida inyectable, además de una entrega más rápida y segura, es capaz de aumentar la previsibilidad de los resultados del tratamiento. Sin embargo, se sabe que las resinas fluidas tienen menor resistencia al desgaste y estabilidad del color en comparación con las resinas convencionales. Así, se introdujo en el mercado una versión de este material con un mayor contenido de relleno en su composición, con el objetivo de mejorar la resistencia al desgaste y la retención del brillo. Se necesitan estudios a largo plazo que evalúen el uso de resinas de baja viscosidad. La técnica inyectable es capaz de reproducir fielmente la morfología obtenida en el encerado del caso, generando un resultado estético y funcional satisfactorio. A pesar de simplificar la fabricación de restauraciones en comparación con la técnica convencional, todavía es necesario que el operador tenga cierta experiencia y aptitud. El tiempo clínico para la producción de restauraciones se ha reducido considerablemente. El material mostró buena fluidez, brillo y pulido en el resultado inmediato. Sin embargo, el acabado y pulido de las restauraciones requirió de un período de tiempo clínico más largo de lo habitual y en el seguimiento a corto plazo, fueron necesarios defectos en los márgenes gingivales y un nuevo procedimiento de acabado y pulido. Por lo tanto, la técnica descrita en este artículo puede considerarse una alternativa restauradora, pero aún no es capaz de reemplazar el método tradicional de realizar restauraciones estéticas previas.

ABSTRACT

A estética tem um papel importante na aceitação e autoestima das pessoas e, em virtude disso, os pacientes buscam cada dia mais ter um sorriso harmonioso. As reabilitações estéticas devem envolver um planejamento completo, atrelando função e estética, avaliando tamanho dos dentes, perfil e alturas gengivais, altura do sorriso e corredor bucal. Por conta disso, a odontologia nos permite uma série de abordagens terapêuticas diferentes que chegam a resultados satisfatórios para o paciente. Sendo assim, o presente trabalho tem como objetivo descrever um relato de caso de uma paciente com queixa estética do seu sorriso devido à diferentes tonalidades, formas e tamanhos dos dentes. Após anamnese, exame clínico e radiográfico, o plano de tratamento proposto foi de harmonizações periodontal com aumento de coroa e enxerto gengival, além de coroas em cerâmicas de dissilicato de lítio. Dentro desse contexto, mostra-se que uma abordagem multidisciplinar para reabilitação estética e funcional do sorriso é fundamental, a qual nos proporciona equilíbrio e naturalidade entre estética branca e vermelha no resultado do tratamento reabilitador(AU)


Aesthetics plays an important role in people's acceptance and self-esteem and, as a result, patients increasingly seek to have a harmonious smile. Aesthetic rehabilitations must involve a complete planning, linking function and aesthetics, evaluating tooth size, gingival profile and heights, smile height and buccal corridor. Because of this, dentistry allows us a series of different therapeutic approaches that reach satisfactory results for the patient. Therefore, the present work aims to describe a case report of a patient with an aesthetic complaint of her smile due to different shades, shapes and sizes of teeth. After anamnesis, clinical and radiographic examination, the proposed treatment plan was periodontal harmonization with crown augmentation and gingival graft, in addition to lithium disilicate ceramic crowns. Within this context, it is shown that a multidisciplinary approach to the aesthetic and functional rehabilitation of the smile is fundamental, which provides us with balance and naturalness between white and red aesthetics in the result of the rehabilitation treatment(AU)

Introdução:O aprimoramento das resinas compostas nos últimosanos em associação com a difusão de informações nas redes sociais tornou as facetas diretas tratamentos populares na dentística restauradora. No entanto, são procedimentos que exigem ampla destreza manual e conhecimento técnico. O fluxo digital através doescaneamento, enceramento digital e prototipagem 3D para construção de guias tem se tornado uma excelente alternativa para aumentar a previsibilidade e aumentar a longevidade destes trabalhos. Objetivo:Descrever o protocolo de confecção de facetas diretas em resina composta, através de um relato de caso, utilizando como auxílio o planejamento digital para confecção de modelo 3D, guia de silicone e paredes palatinas. Descrição do Caso:Paciente do gênero masculino, 43 anos, queixava-se do formato dos seus dentes. Ao exame clínico percebeu-se desgaste dental nos incisivos centrais e linha do sorriso levemente invertida. Após duas sessões de clareamento de consultório com Peróxido de hidrogênio (35%) e mockup direto com resina composta, foi realizada a moldagem e escaneamento do modelo de gesso no laboratório. O enceramento digital foi aprovado, o modelo 3D foi impresso para confecção da guia de silicone. Com auxílio da guia foram executadas facetas diretas nos elementos 13, 12, 11, 21, 22 e 23. Conclusão:O fluxo digital pode ser uma alternativa viável para minimizar as falhas na confecção de facetas diretas em resina composta (AU).


Introduction:The improvement of composite resins in recent years, together with information disseminated on social media, has made direct veneers popular treatments in restorative dentistry. However, these procedures require significant manual dexterity and technical knowledge. Digital work flow using scanning, digital wax-up and 3D prototyping for the construction of guides has become an excellent alternative to increase predictability and the longevity of these procedures. Objective:Describe the manufacturing protocol for direct composite resin veneers, using a case report and digital to construct the 3D model, silicone guide and palatine walls. Case description:Male patient, 43 years old, complained of the shape of his teeth. Clinical examination revealed tooth wear on the central incisors and a slightly inverted smile line. After two whitening sessions with hydroigen peroxide (35%) and direct mockup with composite resin, the plaster model was molded and scanned in the laboratory. Digital wax-up was approved, and the 3D model was printed to manufacture the silicone guide. With the help of the guide, the direct veneers were applied to elements 13, 12, 11, 21, 22 and 23.Conclusions:Digital flow may be a feasible alternative to minimize manufacturing flaws in direct composite resin veneers (AU).


Introducción: La mejora de las resinas compuestas en los últimos años, y la difusión de información en las redes sociales, ha popularizado las facetas directas en los tratamientos en odontología restauradora. Sin embargo, son procedimientos que requieren demasiado destreza manual y conocimientos técnicos. El flujo digital usando escaneo, encerado digital y prototipado 3D para la construcción de guías se ha convertido en una excelente alternativa para aumentar la previsibilidad y la longevidad de estos procedimientos. Objetivo: Describir el protocolo para la realización de carillas directas en resina compuesta, a través de un reporte de caso, utilizando el planeo digital como ayuda para la realización de un modelo 3D, guía de silicona y paredes palatinas. Descripción del caso: Paciente masculino, 43 años, se quejó de la forma de sus dientes. El examen clínico reveló desgaste dental en los incisivos centrales y una línea de sonrisa levemente invertida. Después de dos sesiones de blanqueamiento en consultorio con peróxido de hidrógeno (35%) y maqueta directa con resina compuesta, el modelo de yeso fue moldeado y escaneado en el laboratorio. El encerado digital fue aprovado, el modelo 3D fue impreso para hacer la guía de silicona. Con la ayuda de la guía se realizaron carillas directas en los elementos 13, 12, 11, 21, 22 y 23. Conclusiones: El fluxo digital puede ser una alternativa viable para minimizar fallas en la fabricación de carillas directas en resina compuesta (AU).

ABSTRACT

As lesões dentárias traumáticas são um problema de saúde pública mundial, dentre as quais a fratura dentária é a de maior ocorrência, envolvendo principalmente os dentes anteriores. A abordagem de grandes fraturas anteriores é um desafio para cirurgiões dentistas de qualquer nível pois, além da função, afetam diretamente a estética do paciente, fazendo-se necessário um planejamento que envolva diferentes especialidades na odontologia. Objetivo: Relatar um caso clínico de reabilitação funcional e estética de fratura dentária nos incisivos centrais superiores envolvendo tratamento endodôntico, instalação de pino de fibra de vidro e restauração direta em resina composta. Relato de Caso: O paciente de 17 anos sofreu fratura nos incisivos centrais superiores causada por uma queda de bicicleta. Devido à busca tardia por tratamento o elemento 21 foi diagnosticado com necrose pulpar e, portanto, foi submetido à tratamento endodôntico pela Técnica Crown Down e reabilitação com pino de fibra de vidro Splendor-SAP. Posteriormente, os elementos 11 e 21 foram restaurados com facetas diretas em resina composta pela técnica incremental. Conclusão: A reabilitação com instalação de pino de fibra de vidro associada à técnica de estratificação incremental em resina composta se mostra como uma boa opção para reabilitação estética pois permite dar forma anatômica ao dente com riqueza de detalhes na estratificação da dentina e esmalte, além de máxima preservação da estrutura dental. Os resultados obtidos reforçam o sucesso estético e funcional com significativo impacto na qualidade de vida do paciente(AU)


Traumatic dental injuries are a worldwide public health problem, among which dental fractures are the most frequent, mainly involving the anterior teeth. The approach of large anterior fractures is a challenge for dental surgeons of any level because, in addition to function, they directly affect the patient's esthetics, requiring a plan that involves different specialties in dentistry. Objective: Report a clinical case of functional and aesthetic rehabilitation of dental fractures in maxillary central incisors involving endodontic treatment, installation of a fiberglass post and direct restoration in composite resin. Case Report: The 17-year-old patient suffered a fracture in the upper central incisors caused by a fall from a bicycle. Due to the late search for treatment, element 21 was diagnosed with pulp necrosis and, therefore, underwent endodontic treatment using the Crown Down Technique and rehabilitation with a Splendor-SAP fiberglass pin. Later, elements 11 and 21 were restored with direct veneers in composite resin using the incremental technique. Conclusion: Rehabilitation with the installation of a fiberglass post associated with the incremental layering technique in composite resin is a good option for aesthetic rehabilitation as it allows the anatomical shape of the tooth with rich details in the layering of dentin and enamel, in addition to maximum preservation of the tooth structure. The results obtained reinforce the aesthetic and functional success with a significant impact on the patient's quality of life(AU)

Introdução: A procura pela estética do sorriso é crescente na Odontologia, pois, muitas vezes um sorriso alinhado e periodonto saudável não são o bastante. A cerâmica odontológica apresenta-se como uma ótima possibilidade restauradora, visto o avanço das técnicas de adesão entre o substrato dentário e as cerâmicas, torna-se mais previsível o resultado com este tipo de reabilitação. Objetivo: O presente artigo tem como objetivo descrever um caso clínico com envolvimento de diversos níveis de desgaste dentário para reabilitação estética com cerâmica de dissilicato de lítio. Relato de caso: Paciente 36 anos, gênero feminino, apresentava insatisfação com seu sorriso devido a forma e a alteração de cor dos seus dentes superiores anteriores e pré-molares. O plano de tratamento consistiu em realizar a troca da coroa metalocerâmica por coroa total cerâmica da unidade 14, coroa total cerâmica nas unidades 11, 21 e 22, facetas cerâmicas nas unidades 15, 13, 12, 23, 24 e 25, osteotomia e gengivoplastia das regiões de 2º prémolar superior esquerdo ao 2º pré-molar superior direito, clareamento dental caseiro da arcada inferior e placa oclusal superior de proteção. O material de escolha para a resolução do caso clínico foi a cerâmica vítrea reforçada à base de dissilicato de lítio, pois, possui um índice de refração de luz semelhante à estrutura dentária, sem interferência significativa da translucidez, além de ser passível de cimentação adesiva por se tratar de uma cerâmica ácido sensível e o tamanho do cristal e a sua disposição favorecem o aumento das propriedades mecânicas da restauração. Conclusão: O plano de tratamento realizado permitiu o restabelecimento da função e estética, com reprodução das características dos dentes naturais e promoção da jovialidade e da harmonia no sorriso(AU)


Introduction:The demand for smile aesthetics is growing in Dentistry, because, often a smile aligned and healthy periodontium are not enough. Dental ceramics, presented as an excellent restorative possibility, since the advancement of adhesion techniques between the dental substrate and the ceramics, becomes more predictable the result with this type of rehabilitation. Objective: This article aims to describe a clinical case involving several levels of dental wear for aesthetic rehabilitation with ceramics. Case Report: A 36-year-old female patient presented dissatisfaction with her smile due to the shape and color alteration of her anterior and premolar upper teeth. The treatment plan consisted of exchanging the metal-ceramic crown for the total ceramic crown of unit 14, total ceramic crown in units 11, 21 and 22, ceramic veneers in units 15, 13, 12, 23, 24 and 25, osteotomy and gingivoplasty of the regions from 2nd left upper premolar to 2nd right upper premolar, home tooth whitening of the lower arch and upper occlusal plate of protection. The material of choice for the resolution of the clinical case was the glass ceramic reinforced on the basis of lithium disilicate because it has a refractive index of light similar to the dental structure, without significant interference of translucency, besides being liable to adhesive cementation because it is a sensitive acid ceramic and the size of the crystal and its disposition favor the increase of the mechanical properties of the restoration. Conclusion: The treatment plan performed allowed the restoration of function and aesthetics, with reproduction of the characteristics of natural teeth and promotion of joviality and harmony in the smile(AU)

ABSTRACT

Mediante este caso se describen los criterios de selección del color en restauraciones indirectas con disilicato de litio en el sector anterior. Mujer de 34 años, atendida en la Facultad de Odontología de la Universidad Nacional de Concepción (FOUNC), Paraguay, por una preocupación estética del sector anterior, en especial la diferencia de color de las piezas dentarias. La rehabilitación consistió en la confección de coronas de disilicato de litio de las piezas 11, 12, 13, 14, 21, 22, 24, 25 y laminado para carillas de la pieza 23. Fueron aplicadas técnicas de selección de color individualizadas, pruebas de color y cementación antes del cementado final para obtener resultados altamente estéticos. Gracias a los resultados fue posible observar la conjunción de factores como la correcta selección del color del sustrato dentario, el cemento resinoso, el material de restauración y la comunicación eficiente con el laboratorio, fundamental para el éxito del tratamiento.


This case describes the color selection criteria for indirect lithium disilicate restorations in the anterior sector. A 34-year-old woman treated at the Faculty of Dentistry of the National University of Concepción (FOUNC), Paraguay, due to an aesthetic concern in the anterior sector, especially the difference in color of the teeth. The rehabilitation consisted of making lithium disilicate crowns for teeth 11, 12, 13, 14, 21, 22, 24, 25 and laminate for veneers for tooth 23. Individualized color selection techniques, color and cementation prior to final cementation for highly esthetic results. Thanks to the results, it was possible to observe the conjunction of factors such as the correct selection of the color of the dental substrate, the resinous cement, the restorative material, and efficient communication with the laboratory, essential for the success of the treatment.

ABSTRACT

An aesthetic smile is usually associated with healthy appearance and success in many areas of life. Currently, individuals access the internet and social media in search of health information. The aim of this study was to analyse the quality and reliability of information in videos available on the YouTubeTM platform about ultra-thin ceramic laminates. Methods: YouTubeTM was searched using the therms "dental ceramic laminates" and "dental contact lens" for videos uploaded on ultra-thin ceramic laminates. The first 100 videos in Portuguese were selected and rated for quality and reliability. The content and source of videos were also noted. The quality of the videos was determined by evaluating the presence or absence of reliable information and distributed into seven domains. To compare the distributions of quantitative variables between the classifications of source, content, and quality, the Kruskal-Wallis and Dunn's post-hoc tests were performed. Results: Seventy-two videos were included for qualitative analysis. Regarding quality rating, most videos were rated with poor quality. The reliability of the evaluated videos was low. Conclusion: YouTubeTM showed a large number of videos with many views on the topic, but it does not contribute with good quality and reliable information to the population

ABSTRACT

Aims: To verify the efficacy of Er,Cr:YSGG laser for debonding of lithium disilicate (LD) reinforced glass ceramic veneers of different thicknesses. Methods: Forty bovine teeth were prepared and randomly divided into four groups (n=10/group) according to the ceramic disc thickness: C0.5 (Control group) and L0.5 (Laser irradiated group) in which LD discs had a thickness of 0.5mm and 5mm diameter; C1 and L1 in which LD discs had a thickness of 1mm and 5mm diameter. The lithium disilcate discs (IPS E.max®, shade HTA2) were fabricated following the manufacturer's recommendations and cemented to the prepared tooth surface. The Er,Cr:YSGG laser was applied to the laser groups at 2.5W and 25Hz for 60seconds. Universal testing machine was used to evaluate the shear bond strength for all samples at a cross head speed of 1mm/min in an inciso-gingival direction parallel to the sample surface. After debonding, the samples were examined under stereoscope to evaluate the mode of failure according to the adhesive remnant index (ARI). Results: Laser irradiation significantly diminishes the shear bond strength from 10.868 MPa to 3.778 MPa for C0.5 and L0.5 groups respectively (p=0.00) and from 14.711 MPa to 4.992 MPa for C1 and L1 groups respectively (p=0.00). The shear bond strength required for debonding increased with increasing thickness of discs, but without significant difference (p=0.110). Higher ARI scores were seen in the laser groups (more cement remaining adhered to the tooth) when compared to the control groups. Conclusions: The Er,Cr:YSGG laser could be an effective and useful tool in debonding of lithium disilicate ceramic veneers as it decreases the shear bond strength required for veneer debonding

ABSTRACT

Abstract To evaluate stress distribution when applying vertical and tangential forces to 1mm thick occlusal veneers with different finish lines preparations, using the finite element method. One extracted third molar was prepared for occlusal veneers, firstly without any bevel. It was scanned in order to design two groups of lithium disilicate (G1A, G2A) and composite resin (G1B, G2B) occlusal veneers. Then, the third molar preparation was modified, beveling the finish line and it was subsequently scanned again to design the occlusal veneer groups with bevel (LD: G3A, G4A and CR: G3B, G4B). The four groups were subjected to different forces (400 N vertical and 900 N tangential). At 400 N, the non-beveled veneers showed slightly higher Von Mises stress values (G1A: 783 MPa and G1B 736.5 MPa) than the beveled veneers (G3A: 685.7 MPa and G3B: 675.8 MPa). However, when 900 N tangential forces were applied, the beveled occlusal veneers showed higher Von Mises stress values (G4A: 4297 MPa and G4B: 4133 MPa) than the non-beveled occlusal veneers (G2A: 2581.1 MPa and G2B: 3519.1 MPa). Furthermore, it was observed that the tissue under the occlusal veneers with bevel showed higher Von Mises stress values than the models without any bevel. Beveled and non-beveled occlusal veneers of lithium disilicate and composite resin presented similar stress distribution values when vertical forces of 400 N were applied; whereas with tangential forces of 900 N applied near to the finish line, the beveled groups presented notably higher stress values than the non-beveled groups. However, both finish line preparations presented adequate values for possible clinical performance.


Resumen Evaluar la distribución de estrés al aplicar fuerzas verticales y tangenciales en carillas oclusales de 1mm de espesor con diferentes líneas de terminación marginal por el método de elementos finitos. Se preparó un tercer molar extraído para carilla oclusal, primero sin bisel. Se escaneó para diseñar dos grupos de carillas oclusales de disilicato de litio (G1A, G2A) y resina compuesta (G1B, G2B). Después, esta preparación molar fue modificada, biselando la línea de terminación y escaneándola nuevamente para diseñar los grupos de carillas oclusales con bisel (DL: G3A, G4A and RC: G3B, G4B). Los 4 grupos fueron sometidos a fuerzas diferentes (400 N vertical y 900 N tangencial). Con 400 N, las carillas sin bisel presentaron ligera mayor tensión de Von Mises (G1A: 783 MPa y G1B 736.5 MPa) que las carillas con bisel (G3A: 685.7 MPa y G3B: 675.8 MPa). De distinta forma, al aplicar fuerzas tangenciales de 900 N, las carillas oclusales con bisel presentaron mayor tensión de Von Mises (G4A: 4297 MPa y G4B: 4133 MPa) que las carillas oclusales sin bisel (G2A: 2581.1 MPa y G2B: 3519.1 MPa). Además, se observó que los tejidos subyacentes a las carillas oclusales con bisel, presentaron mayor tensión de Von Mises, frente a los modelos sin bisel. Las carillas oclusales con y sin bisel de disilicato de litio y resina compuesta presentaron una distribución de estrés similar con fuerzas verticales de 400 N, por otro lado, con fuerzas tangenciales de 900 N, los grupos con bisel presentaron notablemente mayor tensión que los grupos sin bisel. Sin embargo, ambos diseños de terminación marginal presentaron valores adecuados para un posible desempeño clínico.

Abstract The presence of deciduous canines in adulthood may be of aesthetic concern for patients. Many clinical alternatives include surgical and orthodontic treatment. Since patients search for highly esthetic results and more conservative alternatives, the present clinical report describes a treatment for deciduous canine through an indirect composite resin veneer and an indirect composite resin fragment restoration in a premolar to improve esthetic outcomes with a one-year follow up.


Resumen La presencia de caninos deciduos en adultos puede ser de preocupación estética por parte de los pacientes. Diversas alternativas clínicas pueden incluir tratamientos quirúrgicos y ortodóncicos. Desde que los pacientes buscan por resultados de alta estética y cada vez más conservadores, es que el presente caso clínico describe el tratamiento de un canino deciduo mediante una carilla indirecta de resina compuesta junto a un fragmento indirecto de resina de un premolar con el fin de mejorar los resultados estéticos, y con un año de seguimiento.

Introdução: As cerâmicas odontológicas são amplamente utilizadas na Odontologia, desfrutando de grande popularidade devido a sua capacidade de mimetizar as características ópticas do esmalte e da dentina. O constante desenvolvimento de pesquisas tem desencadeado o lançamento de novos sistemas cerâmicos com propriedades aprimoradas e melhores resultados clínicos em longo prazo. Objetivo: O relato de caso aqui apresentado descreve uma reabilitação da zona estética do sorriso com laminados cerâmicos por meio de uma abordagem minimamente invasiva. Relato de caso: Paciente do gênero feminino, compareceu à clínica odontológica relatando insatisfação com a aparência do seu sorriso. Durante avaliação foram observadas, na região anterossuperior, restaurações extensas em resina composta deficientes nos incisivos, com proporção, volume, forma e texturização insatisfatórias, que em conjunto comprometia a zona estética do sorriso. Diante disso, após fotografias extra e intraorais, o tratamento proposto foi a reabilitação com laminados cerâmicos nas unidades 1.2 ao 2.2. Assim, foi realizado o enceramento e o mock-up, os quais serviram de orientação para os preparos dentários, a seguir, foram moldados e as restaurações confeccionadas no sistema IPS e.max. Após prova de cor e adaptação, os preparos e a superfície interna dos laminados cerâmicos foram tratados e cimentados com cimento resino fotopolimerizável. Conclusão: Instituindo uma abordagem conservadora, o plano de tratamento realizado através da reabilitação com laminados cerâmicos nas unidades dentárias anterossuperiores, permitiu o restabelecimento da função e estética, com reprodução das características dos dentes naturais e promoção do equilíbrio, da jovialidade e da harmonia no sorriso(AU)


Introduction: Dental ceramics are widely used in dentistry, enjoying great popularity due to the ability to mimic both enamel and dentin optical characteristics. The constant research development has triggered the launch of new ceramic systems with improved properties and better long-term clinical results. Objective: The case report presented here describes the zone rehabilitation of the aesthetic smile with ceramic laminates through a minimally invasive approach. Case report: Female patient, attended the dental clinic reporting dissatisfaction with the appearance of her smile. During the evaluation, extensive restorations in composite resin were observed in the incisors in the anterosuperior region, with unsatisfactory proportion, amount, shape and texturing, which together compromised the aesthetic zone of the smile. Faced with this, after extra and intraoral photographs, the proposed treatment was the restoration with the ceramic laminates in units 1.2 to 2.2. Thus, waxing and mock-up were performed, which served as guidance for the dental preparations, then they were molded and restorations fabricated in the IPS e.max system. After shade and adaptation proof, the preparations and the inner ceramic laminates surface were treated and cemented with light-cured resin cement. Conclusion: Adopting a conservative approach, the treatment plan carried out through restoration with ceramic laminates on the upper anterior teeth allowed the recovering function and aesthetics, reproducing characteristics of natural teeth and promoting balance, joviality and harmony in the smile(AU)

Resumen Introducción: El presente informe de caso clínico se enfoca en el diagnóstico y tratamiento para la rehabilitación de la dentición temporal en el sector anterosuperior utilizando tecnología CAD-CAM. Objetivo: Proponer una alternativa para mitigar el margen de error y tiempo de trabajo en el paciente pediátrico, reintegrando la estética y funcionalidad en la cavidad oral. Métodos: Se describe el caso de un paciente de sexo masculino de 6 años que acudió a la clínica universitaria de la Universidad UTE con lesiones cariosas extensas en sus dientes anterosuperiores y su rehabilitación a través de la utilización de coronas personalizadas de resina mediante un encerado digital realizado en CAD-CAM. Resultados: El tratamiento efectuado en el niño fue satisfactorio, se evidenció una mejoría en la dimensión vertical e incremento en la estética dental y facial, mejorando su aspecto y autoestima después de la rehabilitación. Conclusión: La utilización del encerado digital en pacientes pediátricos podría ser una alternativa que permita reducir el tiempo de trabajo del tratamiento en una rehabilitación estética dental.


Abstract Introduction: This case report focuses on the diagnosis and treatment for the rehabilitation of the primary dentition in the anterosuperior sector using CADCAM technology. Objective: To propose an alternative to mitigate the margin of error and working time in pediatric patients, reintegrating esthetics, and functionality in the oral cavity. Methods: We describe the case of a 6-year-old male patient who came to the clinic of Universidad UTE with extensive carious lesions in his anterosuperior teeth and his rehabilitation using personalized resin crowns by means of a digital waxup performed in CAD-CAM. Results: The treatment performed on the child was satisfactory, there was an improvement in the vertical dimension and an increase in dental and facial esthetics, improving his appearance and self-esteem after the rehabilitation. Conclusión: The use of digital wax-up in pediatric patients could be an alternative that allows the treatment time to be reduced in esthetic dental rehabilitation.

Os avanços alcançados no desenvolvimento dos materiais restauradores e procedimentos adesivos têm possibilitado a realização de procedimentos minimamente invasivos nas reabilitações orais. Neste contexto, os laminados cerâmicos tornaram-se uma alternativa viável para a resolução de diversos problemas que acometem a função e estética. Assim, este trabalho apresentou como objetivo relatar um caso clínico de reabilitação estética do sorriso com o fechamento de espaços negros, originados a partir de um trauma, seguido de movimentação ortodôntica, através da utilização de laminados cerâmicos. Neste foram descritas as etapas clínicas de diagnóstico, planejamento e execução do procedimento restaurador, observando que o tratamento proposto foi uma alternativa conservadora e eficaz na resolução dos espaços negros existentes, reestabelecendo a estética do sorriso e devolvendo a auto-estima do paciente(AU)


The advances achieved in the development of restorative materials and adhesive procedures have made possible to perform minimally invasive procedures in oral rehabilitation. In this context, ceramic veneers have become a viable alternative for solving several problems that affect function and aesthetics. Thus, this study aimed to report a clinical case of aesthetic smile rehabilitation with the closure of black spaces, originated from trauma, followed by orthodontic movement, through the use of ceramic veneers. In this, the clinical stages of diagnosis, planning and execution of the restorative procedure were described, noting that the proposed treatment was a conservative and effective alternative in the resolution of existing black spaces, reestablishing the aesthetic of the smile and restoring the patient's selfesteem(AU)

OBJECTIVE@#To analyze the marginal roughness and marginal fitness of chairside computer-aided design and computer-aided manufacturing (CAD/CAM) laminate veneers with different materials and thicknesses, and to provide a reference for the clinical application of laminate veneers.@*METHODS@#The butt-to-butt type laminate veneers were prepared on resin typodonts, the preparations were scanned, and the laminate veneers were manufactured by chairside CAD/CAM equipment. The laminate veneers were divided into four groups (n=9) according to the materials (glass-matrix ceramics and resin-matrix ceramics) and thickness (0.3 mm and 0.5 mm) of the veneers, with a total of 36. The marginal topo-graphies of each laminate veneer were digitally recorded by stereomicroscope, and the marginal rough-nesses of the laminate veneers were determined by ImageJ software. The marginal fitness of the laminate veneers was measured by a fit checker and digital scanning and measuring method. At the same time, the mechanical properties of glass-matrix ceramic and resin-matrix ceramic bars (n=20) were tested by a universal testing device.@*RESULTS@#The marginal roughness of 0.3 mm and 0.5 mm glass-matrix ceramic laminate veneers was (24.48±5.55) μm and (19.06±5.75) μm, respectively, with a statistically significant difference (P < 0.001). The marginal roughness of 0.3 mm and 0.5 mm resin-matrix ceramic laminate veneers was (6.13±1.27) μm and (6.84±2.19) μm, respectively, without a statistically significant difference (P>0.05). The marginal roughness of the glass-matrix ceramic laminate veneers was higher than that of the resin-matrix ceramic laminate veneers with a statistically significant difference (P < 0.001). The marginal fitness of 0.3 mm and 0.5 mm glass-matrix ceramic laminate veneers were (66.30±26.71) μm and (85.48±30.44) μm, respectively. The marginal fitness of 0.3 mm and 0.5 mm resin-matrix ceramic laminate veneers were (56.42±19.27) μm and (58.36±8.33) μm, respectively. There was no statistically significant difference among the 4 groups (P>0.05). For glass-matrix ceramics, the flexural strength was (327.40±54.25) MPa, the flexural modulus was (44.40±4.39) GPa, and the modulus of resilience was (1.24±0.37) MPa. For resin-matrix ceramics, the flexural strength was (173.71±16.61) MPa, the flexural modulus was (11.88±0.51) GPa, and the modulus of resilience was (1.29±0.27) MPa. The flexural strength and modulus of glass-matrix ceramics were significantly higher than those of resin-matrix ceramics (P < 0.001), but there was no statistically significant difference in the modulus of resilience between the two materials (P>0.05).@*CONCLUSION@#The marginal roughness of CAD/CAM glass-matrix ceramic laminate veneers is greater than that of resin-matrix ceramic laminate veneers, but there was no statistically significant difference in marginal fitness among them. Increasing the thickness can reduce the marginal roughness of glass-matrix ceramic laminate veneers, but has no effect on the marginal roughness of resin-matrix ceramic laminate veneers.

Background: The debonding of crowns and ceramic veneers with laser is already a reality in the clinic. It presents benefits in comparison to traditional removal with the use of a rotating instrument; however, there is still no consolidated protocol and many professionals use it without the necessary scientific basis. Objective: The aim of this work was to perform a literature review on the debonding of indirect ceramic restorations using the Er:YAG and Er,Cr:YSGG lasers to provide clinical professionals and the scientific community a careful analysis, and also to guiding the use of irradiation at laser for such procedures. Methods: Bibliographic searches were performed in the following databases: Pubmed, Web of Science and Google Scholar. According to the inclusion criteria adopted, twenty-seven clinical and in vitro studies were selected in the period from 2007 to 2021. In the present work, the composition of the crowns and facets, as well as the types of cement and the laser irradiation protocols adopted in the selected studies is detailed. Conclusions: It was concluded that the use of erbium lasers for debonding of indirect ceramic restorations proved to be more selective and conservative when compared to removal with a rotary diamond instrument. Furthermore, it was shown to be in more efficient in debonding different types of ceramics. However, there are great variations in the "debonding" protocols, which emphasizes the need for further studies that seek to standardize the irradiation protocols considering the different clinical situations (AU)


Antecedentes: A remoção de coroas e facetas cerâmicas com laser já é uma realidade clínica e apresenta benefícios em relação à remoção tradicional com uso de instrumento rotatório diamantado; entretanto, ainda não existe um protocolo consolidado e muitos profissionais o utilizam sem o embasamento científico necessário. Objetivos: O objetivo deste trabalho foi realizar uma revisão da literatura sobre a remoção de restaurações cerâmicas indiretas utilizando os lasers Er:YAG e Er,Cr:YSGG para fornecer uma análise cuidadosa aos profissionais clínicos e à comunidade científica, além de orientar o uso da irradiação a laser para tal aplicação. Métodos: As buscas bibliográficas foram realizadas, nas seguintes bases de dados: Pubmed, Web of Science e Google Scholar. De acordo com os critérios de inclusão adotados, foram selecionados vinte e sete estudos clínicos e in vitro no período de 2007 a 2021. No presente trabalho, a composição das coroas e facetas, bem como os tipos de cimento e os protocolos de irradiação laser adotados nos estudos selecionados foram detalhados. Conclusão: Concluiu-se que o uso dos lasers de érbio para remoção de restaurações cerâmicas indiretas mostrou-se mais seletivo e conservador quando comparado à remoção com instrumento rotatório diamantado, além de ser eficiente na remoção de diferentes tipos de cerâmicas. No entanto, existem grandes variações nos protocolos de "debonding", o que enfatiza a necessidade de novos estudos que busquem uma padronização dos protocolos de irradiação considerando as diferentes situações clínicas.(AU)

Este relato descreve o caso de uma paciente jovem que necessitou de reabilitação na região maxilar anterior devido a insatisfações estéticas. O artigo objetivou relatar a sequência clínica da reabilitação de um implante pré-existente na região do dente 21 com UCLA de zircônia personalizável e coroa de dissilicato de lítio associada a instalação de facetas de dissilicato de lítio dos dentes 15 ao 25 para harmonização do sorriso. Previamente ao tratamento reabilitador, foi realizado o planejamento do caso que incluiu a necessidade de gengivectomia e condicionamento do perfil de emergência na região peri-implantar. Após a cicatrização, foram confeccionados preparo minimamente invasivos de acordo com as informações do mock-up. UCLA personalizável de zircônia foi selecionada como componente protético, visando a estética do caso. Para a moldagem, utilizou-se transfer de moldeira aberta para cópia do perfil de emergência e posicionamento do implante na região do 21 e moldagem simultânea com silicone de adição. Após a prova, a cimentação das facetas de dissilicato de lítio foi realizada com cimento adesivo fotopolimerizável. Os resultados estéticos e funcionais foram satisfatórios. No presente caso clínico, conclui-se a importância da abordagem multidisciplinar para alcançar resultados satisfatórios. Ademais, a associação das facetas e a coroa de dissilicato de lítio implantossuportada com UCLA de zircônia personalizável proporcionou os resultados estéticos e funcionais da paciente(AU)


This report describes the case of a Young patient who needed rehabilitation in the anterior maxillary region due to esthetic involvement. The article aimed to report the clinical sequence of the rehabilitation of a pre-existing implant in the region of tooth 21 with UCLA of zirconia customizable and crown of lithium disilicate associated with the installation of veneers of lithium disilicate of teeth 15 to 25 for harmonizing the smile. Prior to the rehabilitation treatment, case planning was carried out, which included the need for gingivectomy and conditioning of the emergency profile in the peri-implant region. After healing, preparations minimally invasive were made according to the information of mock-up. UCLA of zirconia customizable was selected as a prosthetic component, aiming at the aesthetics of the case. For impression, open tray transfer was used to copy the emergency profile and position the implant in the region of 21 and simultaneous impression with addition silicone. After the test of veneers, the cementation of veneers of lithium disilicate was performed with adhesive cement light-curing. The esthetic and functional results were satisfactory. In the present clinical case, the importance of a multidisciplinary approach to achieve satisfactory results is concluded. In addition, the combination of veneers and crow implanted of lithium disilicate with customizable zirconia UCLA provided the patient's esthetic and functional results(AU)

Abstract In the RLT (Rapid Layer Technology), veneering ceramic and framework are fabricated by computer-aided design/computer-aided manufacturing (CAD/CAM) and then cemented to obtain the restoration. This study aimed to evaluate the effect of the thickness of veneering ceramic manufactured by the RLT technique on the fracture resistance (FR) of bilayer crowns with zirconia frameworks. Twenty zirconia frameworks and twenty feldspathic posterior crowns with two different veneering ceramic occlusal thicknesses (1mm=TF1; 2mm=TF2) were manufactured using CAD/CAM system. The specimens were luted to an epoxy resin abutment with resin cement and mechanically cycled (200N and 4.5×105 Pa, 37°C, 2×106 cycles, 3Hz). The FR test was performed (10kN, 0.5mm/min), and the specimens were analyzed in a stereomicroscope. For the stress analysis (finite element analysis, FEA), a 10kN load was equal to the in vitro test, and the principal stress was evaluated. The FR data were analyzed by Student's t-test and Weibull's analysis. The thickness influenced the FR of bilayer crowns. The FR was higher in the TF2 than in the TF1 group. The TF2 group presented the highest characteristic strength compared to the group TF1. The predominant type of failure was delamination. The FEA showed higher stress concentrations below the loading application point at the veneering cement interface in the 1-mm-thick model. The bilayer crowns manufactured using the approach of 2mm of veneering ceramic promoted higher FR compared to the group with 1mm veneering ceramic. Also, the FEA showed that the veneer ceramic thickness has an effect on stress distribution in zirconia-based bilayer crowns.


Resumo Na RLT (Rapid Layer Technology), a cerâmica de cobertura e infraestrutura são fabricados pelo Computer-Aided Design / Computer-Aided Manufacturing (CAD / CAM) e cimentados para obter a restauração. Este estudo teve como objetivo avaliar o efeito da espessura da cerâmica de cobertura fabricada pela técnica RLT na resistência à fratura (RF) de coroas bilaminadas com infraestrutura de zircônia. Vinte infraestruturas de zircônia e vinte coroas posteriores feldspáticas com duas espessuras oclusais da cerâmica de cobertura (1mm = TF1; 2mm = TF2) foram fabricadas usando o sistema CAD / CAM. Os espécimes foram cimentados em preparos de resina epóxi com cimento resinoso dual e ciclados mecanicamente (200N e 4,5×105 Pa, 37° C, 2×106 ciclos, 3Hz). O teste de RF foi realizado (10kN, 0,5mm / min) e, posteriormente, os espécimes foram analisados em estereomicroscópio. Para a análise de tensão (análise de elementos finitos, FEA), uma carga de 10kN foi aplicada igual ao teste in vitro, e a tensão principal foi avaliada. Os dados de RF foram analisados pelo teste t de Student e análise de Weibull. A espessura mostrou forte influência na RF das coroas bilaminadas. A RF foi maior em TF2 do que no grupo TF1. O grupo TF2 apresentou a maior resistência característica em relação ao grupo TF1. O tipo de falha predominante foi a delaminação. O FEA mostrou maiores concentrações de tensões abaixo do ponto de aplicação da carga, na interface cimento e cerâmica de cobertura no modelo de coroa de 1 mm de espessura. As coroas de bilaminadas confeccionadas com 2 mm de cerâmica de cobertura promoveram maior RF em comparação ao grupo com cerâmica de cobertura de 1 mm. Além disso, a FEA mostrou que a espessura da cerâmica de cobertura tem um efeito na distribuição de tensões em coroas bilaminadas à base de zircônia.

RESUMEN De acuerdo al modelo patológico multifactorial, la morfología dental ha sido asociada como uno de los factores etiológicos de la caries al favorecer el acumulo de restos alimenticios y la retención de biopelícula. Uno de los rasgos morfológicos dentales más frecuentes de la población colombiana es el punto P del protostílido, el cual se constituye en una fosa que se expresa en el surco de desarrollo vestibulomesial de los molares inferiores, región que le sigue a la superficie oclusal como uno de los sitios en donde se desarrollan con más frecuencia lesiones cariosas. Sin embargo, el desconocimiento de este rasgo morfológico por la mayoría de los odontólogos hace que el sistema morfológico del protostílido sea mal diagnosticado, lo que conlleva en muchos casos al sobretratamiento de dicha fosa con terapéuticas invasivas, las cuales, podrían ser evitadas con un conocimiento adecuado de la morfología dental y con un manejo preventivo o con técnicas no invasivas. Por tanto, el objetivo de esta revisión de tema es reconocer la expresión del punto P del protostílido y realizar una aproximación a las implicaciones clínicas del mismo y las posibilidades diagnósticas y terapéuticas conservadores que ofrece la odontología para controlar el acumulo de restos alimenticios y la retención de biopelícula.


ABSTRACT According to the multifactorial pathological model, dental morphology has been associated as one of the etiological factors of caries by favoring the accumulation of food remains and biofilm retention. One of the most frequent of non-metric dental traits of the Colombian population is the P point of the protostylid, which is constituted in a fossa that is expressed in the bucomesial development groove of the lower molars, a region that follows the occlusal surface as one of sites where carious lesions develop more frequently. However, the lack of knowledge of this morphological feature by most dentists makes the morphological system of the protostylid misdiagnosed, which in many cases leads to the overtreatment of this pit with invasive therapies, which could be avoided with a knowledge adequate dental morphology and with a preventive management or non-invasive techniques. Therefore, the aim of this review of the subject is to reconcile the expression of the P point of the protostylid and to make an approximation to the clinical implications of the same and the conservative diagnostic and therapeutic possibilities offered by dentistry to control the accumulation of food rests and retention of dental biofilm.

ABSTRACT

Abstract Amelogenesis imperfecta (AI) refers to a group of rare genetic disorders that involve tooth development and are passed down through families. Hypoplasic AI phenotypes include the absence of enamel as a result of a defect in the secretory stage. This case report describes the diagnosis and treatment of a patient with hypoplastic AI. The clinical implications include sensitive teeth, functional problems, and aesthetic complaining. The diagnosis was done through history, clinical examination and imaging. The intervention was performed by Direct Resin Veneers. This treatment showed to improve occlusion, esthetics, and self-image of the teenager. The satisfactory clinical result has made it possible to avoid more invasive and expensive treatments.


RESUMEN La amelogénesis imperfecta (AI) se refiere a un grupo de trastornos genéticos raros que involucran el desarrollo de los dientes y se transmiten de padres a hijos. Los fenotipos de AI hipoplásicos incluyen la ausencia de esmalte como resultado de un defecto en la etapa secretora. Este reporte de caso clínico describe el diagnóstico y tratamiento de un paciente con AI tipo hipoplásica. Las implicaciones clínicas incluyen dientes sensibles, problemas funcionales y quejas estéticas. El diagnóstico se realizó mediante anamnesis, exploración clínica e imagenología. La intervención fue realizada con carillas directas de resina. Este tratamiento demostró mejoras en la oclusión, la estética y la autoimagen del adolescente. El resultado clínico satisfactorio permitió evitar tratamientos más invasivos y costosos.
